What Cpt Code Is Used For Marriage Counseling?

The 90847 and 90846 CPT codes are commonly used by therapists for billing couples and families receiving counseling services. These codes are small codes that therapists use to tell insurance about the diagnosis and treatment of mental illness. When checking coverage, it is important not to ask if the provider covers couples or family counseling.

The most common CPT codes for marriage and family therapy include 90837 for individual psychotherapy lasting 60 minutes, 90853 for group psychotherapy, and 90847 for ongoing couples therapy or marriage and family therapy for two clients. Insurance is not typically required to cover marriage counseling, but therapists can bill Medicare independently for their services furnished for the diagnosis and treatment of mental illnesses.

In 2023, Congress mandated that Medicare provide coverage and payment for the services of health professionals who are Marriage and Family Therapists (MFTs) and Mental Health Counselors (MHCs). TRICARE only covers marriage counseling when necessary for the treatment of a diagnosed mental health condition.

Add-on CPT codes like 99355 and 90840 are used for additional time after the first 60 minutes, 30 to 74 minutes, and 30 minutes of psychotherapy for crisis. CPT code 90847 is specifically used for couples and family counseling when the client is present, including counseling for relationship issues, parenting, and other mental health concerns.
