Medicare now recognizes Marriage and Family Therapists (MFTs) and Mental Health Counselors (MHCs) as qualified behavioral health providers. Section 4121 of Division FF of the Consolidated Appropriations Act, 2023 (CAA, 2023) establishes a new Medicare benefit category for MFTs and MHCs. As of January 1, 2024, an estimated 400,000 MFTs and MHCs are now eligible to enroll as Medicare providers. Medicare Part B covers 80% of the cost of counseling therapy if it is provided by a behavioral health care provider, but not by a licensed MFT or MHC.
MFTs and MHCs can start the Medicare enrollment process prior to January 1, 2024, but billing privileges cannot be effective. Medicare Part B covers individual and group psychotherapy with doctors or other licensed professionals that your state allows, as well as a variety of other mental health services, such as depression screening, diagnostic tests, and family counseling.
Marriage and family therapists are typically not covered by the Medicare program unless they hold certifications and licensures that include them in one of the eligible categories or work for a provider that is eligible for Medicare coverage. A Bipartisan Policy Center task force recommended that Congress expand the mental health provider types covered under Medicare to address shortages in rural areas while Medicare.
In addition to the new Medicare coverage, Part B pays a portion of marital and family counseling costs to treat mental health. As of January 1, 2024, therapists and counselors will finally be eligible as Medicare providers.
